Title: Improving Healthcare Quality and Reducing Errors: The Role of Insurance Companies and Provider Incentives
Landmark studies To Err is Human (1999) and Crossing the Quality Chasm (2001) from the Institute of Medicine (IOM) identified medical errors are causing harm and death to almost 45,000 Americans and costing over $29 billion every year. Other studies have indicated preventable healthcare associated conditions (HACs) such as infections, sepsis, CHF, and pneumonia are main contributors for increasing the length of patient stays, the cost of care, and the likelihood of mortality (death).

Consider the below case study:
Margret Spinner-Ramirez is a 66 y/o female Hispanic-American who speaks both Spanish and English fluently. She is retired, lives alone, and has Medicaid as her primary insurance. Ms. Spinner-Ramirez was scratched by a stray cat that she feeds on her back porch daily. She has been cleaning the wound daily; however, after 2 days she went to her local ER for increased pain, redness, and swelling in her left lower leg wound where she was scratched by the stray cat. Ms. Spinner-Ramirez explained she recently had her left knee replaced about 6 months ago and verbalized new difficulty with baring weight on that extremity to the point that she was having to use her cane again. Her vital signs at the ER visit were stable. Her left leg wound was cleaned and redressed. After 5 hours in the ER, Ms. Spinner-Ramirez was discharged to home on oral Keflex for her left lower leg infection and was instructed to call her primary care physician for a follow-up appointment. Five days later Ms. Spinner-Ramirez was taken back to the same ER via ambulance. Her neighbor found her lethargic, short of breath, and was experiencing difficulty being able to move. A CT scan and blood work revealed that Ms. Spinner-Ramirez’s knee replacement in her left leg was infected secondary to the cat scratch. She was admitted as an inpatient for sepsis. She needed to have a second left knee replacement surgery with wound irrigation and debridement, which extended her inpatient stay to 3 weeks. Once she was discharged to home, she required six weeks of IV antibiotics, extensive rehab, and home health.

ASSIGNMENT OVERVIEW
Identify one vulnerable population within a selected community. Explore current literature and related data to better understand the variables that place this population at risk for health concerns and health disparities. Analyze national health directives (such as Healthy People or another national initiative) to determine correlation with needs/risks/disparities of the vulnerable population.
Part 1: Concept Map
1. Identify the following elements to include in the concept map.
a. Vulnerable population
b. Variables (risk factors) that place the identified population at risk (minimum of three)
c. Minimum of one health risk or disparity for which the population is at risk
2. Create a concept map that portrays each element and the relationships between the vulnerable population, variables placing the population at risk, and the identified health risk or disparity for which the population is susceptible. The concept map must be your own, original work.
Part 2: Summary Paper
In a 1-3 page paper, address the elements listed below. Include a minimum of four scholarly sources, current within 5 years, to support your work. See the rubric for specific, required content criteria within each section of the paper.
1. Provide an introduction to the paper.
2. Include a succinct description of the vulnerable population identified, a minimum of three variables that place the population at risk, and the identified health risk(s) or disparity that the population is at risk of experiencing. Explain how the concept map portrays the relationships between each element:
a. Vulnerable population
b. Variables that place the vulnerable population at risk (minimum of three)
c. Health risks and disparities for which the population is at risk (minimum of one)
3. Identify one national population health goal or objective that relates to the identified risk or disparity (such as Healthy People or another national initiative). “Exploring the TQE Method in Understanding and Managing Eczema”
Class participation and engagement are very important in online
programs. Every student must post a thread using the TQE method on the discussion board about the given topic. The topic is provided below. Each student
must have 1 post. Must have a minimum of 1 reference for your post. Reference must be in APA format and published within the last five
years.
Ever heard of the TQE method?
This protocol has students come up with their own Thoughts,
lingering Questions, and Epiphanies from an assigned reading. Consider
posting some of the “notes” you might want to remember from reading in
outline form (like an electronic note card} with the most important
thing about _______(certain disorder in module) as your thoughts. Then
post some question about this disorder you might still have and then the
epiphany (the must know biggest thing to remember about the disorder).
The epiphany should answer your own questions. Short sweet and to the
point! Pick something to write about that is not the easiest for you to
understand and by writing about it you move it to a deeper remembering
place in your brain.
Topic:
Managing Eczema
Eczema has increased over the last several decades, currently affecting up to 25% of children and 2% to 3% of adults. Atopic dermatitis is the most
common type of eczema
Eczema triggers differ between patients and can change with age. A careful history can help determine each patient’s triggers.
Avoiding triggers is important to reduce the frequency and severity of eczema flares. -
