In a minimum of seven pages, discuss the five leadership domains assessed by the ACHE assessment tool. Case Study 3: Holistic Management of Type 2 Diabetes with Comorbidities
Patient Profile: Sarah, a 55-year-old woman, presents to her primary care nurse practitioner
with complaints of increased thirst, frequent urination, and fatigue over the past few weeks.
She has a family history of type 2 diabetes mellitus (T2DM), with her father and two siblings
diagnosed with the same condition. She has a BMI of 30.9 and a sedentary lifestyle. Laboratory
tests reveal elevated fasting blood glucose levels of 140 mg/dL, an HbA1c of 8.5%, and a total
cholesterol level of 207 mg/dL, and triglycerides of 158 mg/dL Her blood pressure measures is
135/78 mmHg,
Case Scenario: Sarah is your patient now, and you have just diagnosed her with type 2 diabetes.
You (as her PCP) initiate a comprehensive treatment plan according to the latest guidelines to
mitigate long-term complications.
Questions:
1. According to the latest guidelines, what would be the initial pharmacologic and nonpharmacological treatment for Sarah’s type 2 diabetes?
2. According to the latest guidelines, what additional preventive (pharmacological and nonpharmacological) measures would need to be taken to mitigate Sarah’s risk of cardiac
consequences, considering her lipids and hypertension state?
3. Which referrals, if any, would be necessary for Sarah’s comprehensive management, and
what specific assessments or interventions would these referrals entail?
4. When would you recommend scheduling Sarah for a follow-up appointment to assess
her response to treatment ?

Given the above scenario, please answer the following questions:
1. Mary feels her concerns regarding the shortage of beds for psychiatric patients in her county are falling on deaf ears. As a result she has turned to her professional nursing organization to help her. Discuss the importance of professional nursing organizations and how registered nurses can utilize these groups to improve care.

Students much review the case study and answer all questions with a scholarly response using APA and include 2 scholarly references. Answer both case studies on the same document and upload 1 document to Moodle.

History of Present Illness (HPI): Randi, a 22-year-old female, presents to the clinic with c/o of a 3-day
history of thick white vaginal discharge, intense vaginal itching, and dysuria. She reports she is sexually
active with 1 partner. No history of STI’s. She had a recent sinus infection and was on amoxicillin x 10
days.
Prior medical history: None.
Prior surgical history: None.
Current medications: Mirena IUD – inserted last year. Allergies: Sulfa
Social history: College student. Lives with mother. Denies smoking or recreational drugs. Vapes daily.
Family history: Mother alive and well. Father alive and well. Sister – diabetes: uses insulin pump
OB- GYN History: Menarche age 13, cycle length 5 days – frequency every 28 days. No history of
sexually transmitted infections (STIs). Never had a pap smear.
Review of Systems (ROS): As noted in HPI.
Physical Exam (PE)
VS: BP: 110/70, P: 109, RR: 17, T: 98.9, Weight: 132 lbs., Height 65 inches, BMI 22 kg/m2
• General: AAO x 3, pleasant.
• Gynecological: EXTERNAL EXAM: mild erythema, white clumpy discharge. SPECULUM/INTERNAL
EXAM: Cervix: normal appearance, no lesions, no bleeding, white discharge, no cervical
movement tenderness. UTERUS: normal size, shape, and consistency, normal mobility,
nontender. ADNEXA: no masses or tenderness bilaterally.
