“Integrating Culture and Humanistic Care: Application of Leininger’s Sunrise Model in Clinical Practice”
The Leininger ‘Sunrise’ model focuses on culture and humanistic care as essential components of all societies. The Leininger model contains three (3) main intervention strategies or approaches. The goal is that the nurse will be continuously mindful of these three interventional strategies and apply them in patient care settings often when, and where appropriate. The three intervention strategies from the model are:
Preservation/maintenance (support / reaffirm patient cultural preference / the situation may remain the same / document the cultural support afforded the patient).
Accommodation/negotiation (support / reaffirm patient cultural preference but compromise must occur/document the agreed-on compromise).
Re-patterning/restructuring (support / but a patient behavioral change needs to occur/document the changed behavior/intended outcome. Note, that ‘teaching’ is not enough. The nurse needs to identify the outcome of the teaching provided/did a change occur).
Write up 3 actual/real examples they used in their clinical setting – one (and only one) example for each of the 3 modes. Section is to be 450-550 words
Culture Care Preservation and/or Maintenance: State what specific culture this patient identifies with and what cultural issue/challenge/need is being addressed in this example: (fill in the example)
Culture Care Accommodation and/or Negotiation: State what specific culture this patient identifies with and what cultural issue/challenge/need is being addressed in this example: (fill in the example)
Culture Care Re-patterning and/or Restructuring: State what specific culture this patient identifies with and what cultural issue/challenge/need is being addressed in this example: (fill in example)
Remember, for ‘re-patterning’ saying you ‘taught’ the patient about a needed change, is not enough. You must also report if the patient agreed, or did not agree, to do what you suggested/asked of them. In other words, was there a change? -
“Complex Case Presentation: PTSD and GAD in a Traumatized Adolescent”
Gabriella, thanks for your complex case presentation.
PTSD is a condition that can developed as early as a month after experiencing or witnessing a stressful incident, as emphasized by Shalev et al. (2017). The individual exhibits symptoms such as sleep and concentration difficulties, engaging in self-harm, avoiding triggers related to trauma, experiencing irritability, losing interest in pleasurable activities, guilt for the traumatic event, dissociation, having distressing dreams or nightmares, engaging in reckless behavior, being unable to express positive emotions, and displaying thoughts or behaviors related to the trauma. In this patient’s case, he is showing several PTSD symptoms that, according to DSM 5 criteria, a provider can correctly diagnose the patient with. As a secondary differential diagnosis, I concur with you any significant event or trauma can lead to a diagnosis of GAD; again, this patient has experienced one of the most traumatic events, like witnessing the death of his father and subsequently moving to the United States, establishing relationships with new friends, possibly meeting new family members and attending a new school can all be contributing to the second differential. GAD is associated with multiple symptoms that the patient is displaying. Irritability, lack of concentration, and sleeping disturbances. (James et al., 2020).

Capstone Part I: Exploring the Impact of Legal Death with Dignity Regulations on Terminally Ill Patients’ Experiences of Suffering, Autonomy, and Quality of Life in Texas
Capstone Part I
Overview:
The Capstone Paper provides students an opportunity to actively integrate and apply concepts they have learned to the development and analysis of a project that addresses a problem or issue encountered within the student’s profession, institution, or organization. The student will incorporate clinical reasoning and evidence-based practice using a systematic process for the ultimate goal of comprehensive, safe patient care. Each individual assignment in the synthesis course will build to a larger capstone project due near the end of the course. In the introduction assignment, the student will discuss the introduction, background, and significance of a chosen clinical topic and develop a PICOT-formatted clinical question.
Submit the assignment by the due date on the calendar as a .docx in APA format.
Part I of the paper is organized in a template according to the following sections. Please – use this APA-formatted template!
Introduction
Background and Significance
PICO(T) evidence review question
Literature Review
The entire capstone paper must include a minimum of 5 articles, excluding websites and blogs. Students are encouraged to include as many as references as needed to support the project.
NOTE THE TEMPLATES ATTACHED-You should have the initials files you worked on for this project
The Subject to write about:
In terminally ill patients in Texas receiving care in palliative and hospice settings, how does the absence of legal Death with Dignity regulations, compared to standard palliative care, affect patients’ experiences of suffering, maintenance of patient autonomy, and quality of life in the final months of life?
Considering your Capstone II paper where you will be choosing a study design, and implement the capstone consider a comparative cohort study design.
Here’s why:
Population: You will have two groups of terminally ill patients in Texas: those receiving care in settings with legal Death with Dignity regulations (if any) and those receiving standard palliative care.
Comparison: The study will compare these two groups on the basis of their experiences of suffering, maintenance of autonomy, and quality of life.
Outcome Measurement: These outcomes can be measured using validated questionnaires and scales over the last months of the patients’ lives.
Here are some validated questionnaires to consider:
Validated tools to measure patient experiences of suffering (e.g., pain scales, symptom burden questionnaires)
Autonomy (e.g., autonomy scales, patient interviews)
Quality of life (e.g., Quality of Life at End of Life questionnaire).
