PurposeThe purpose of this discussion is to introduce your peers to your specific practice problem at the practicum site. Please consider your discussion with organizational key stakeholders, how the problem was identified, and the practice gap that informs your project design. (WORD DOCUMENT ATTACHED)
InstructionsReview this week’s readings and provide your response in 150 words or less:
State your practice question in PICOT format. (MY PICOT- In adult patients diagnosed with type 2 diabetes at a primary care clinic, does implementation of the CDC Diabetes Prevention Program, compared to current practice, impact fasting blood glucose, over 8-10 weeks?)
State the title of your Project: The title should inform and engage the reader and must be succinct. The title should include the intervention and the anticipated outcome and may include the population. (Please do not include abbreviations in the title.) (COULD THIS BE MY TITLE? – Effectiveness of the Diabetes Prevention Program on fasting blood glucose in type 2 diabetes)
Examine the problem at the practicum site and evaluate the gap in practice you will address with this project. To examine the problem at the practicum site and evaluate the gap in practice, you would first have to do the following:Identify the Problem: Begin by clearly identifying the problem or issue at the practicum site. This could be related to inefficiencies, gaps in service delivery, outdated processes, or any other challenge that hinders the optimal functioning of the organization (Bemker, Ralyea, Schreiner, 2021).
Conduct a Needs Assessment: Perform a needs assessment to gather information on the current state of affairs. This may involve interviews, surveys, observations, and a review of relevant documents (Bemker, Ralyea, Schreiner, 2021). Understand the perspectives of different stakeholders to get a comprehensive view.
Analyze Current Practices: Examine the existing practices and processes that contribute to or exacerbate the identified problem (Bemker, Ralyea, Schreiner, 2021). Look for patterns, root causes, and any systemic issues that may be contributing to the gap in practice.

Title: Preventing Workplace Violence in Healthcare Settings: A Critical Analysis of Interventions and Policies
Topic: Workplace Violence in Healthcare Settings
Learning Objectives:
Critically analyze research related to workplace violence in healthcare.
Evaluate the effectiveness of interventions and policies to reduce violence against healthcare workers.
The Scenario:
Healthcare workers face a significant risk of violence from patients, families, and visitors. This violence can take many forms, ranging from verbal abuse and threats to physical assault. This assignment will utilize the Cochrane Library, a trusted source of high-quality healthcare evidence, to explore research on this critical issue.
The Task:
Search Strategy: Access the Cochrane Library (https://www.cochranelibrary.com/Links to an external site.). Develop a focused search strategy using relevant MeSH terms (Medical Subject Headings) and keywords to identify systematic reviews addressing workplace violence in healthcare settings.
Here are some MeSH terms and keywords to get you started, but feel free to modify and expand your search:
MeSH: Workplace Violence, Violence, Healthcare Personnel, Nurses, Hospitals
Keywords: Assault, Aggression, Verbal Abuse, De-escalation Techniques, Workplace Safety Interventions
Review Selection: Apply inclusion and exclusion criteria to refine your search results. For example,
Include Systematic reviews published in the past 5 years, focusing on interventions to prevent violence against healthcare workers.
Exclude: Review articles not published in peer-reviewed journals, studies focusing on violence outside of healthcare settings.
Critical Appraisal: Select two relevant systematic reviews from the Cochrane Library. For each review, critically appraise the research methodology using tools like the AMSTAR 2 (https://www.bmj.com/content/358/bmj.j4008Links to an external site.). Focus on aspects like the quality of included studies, risk of bias assessment, and the strength of the evidence presented.
Synthesis and Recommendations: Write a concise report summarizing the findings of the two chosen reviews. Highlight the effectiveness of different interventions to prevent workplace violence in healthcare settings. Based on this evidence, propose strategies that nursing leaders can implement to create a safer work environment for nurses and other healthcare professionals.
Deliverables:
A search strategy used in the Cochrane Library.
A brief summary of the inclusion and exclusion criteria for selecting reviews.
A critical appraisal of each chosen systematic review using a chosen tool.
A report summarizing the main findings and recommendations for promoting workplace safety in healthcare. -
Pulmonary Function and Fluid, Electrolyte and Acid-Base Homeostasis Title: Managing Pulmonary Function and Fluid, Electrolyte, and Acid-Base Imbalances in Two Patients Case Study 1: Pulmonary Function
Pulmonary Function:
D.R. is a 27-year-old man, who presents to the nurse practitioner at the Family Care Clinic complaining of increasing SOB, wheezing, fatigue, cough, stuffy nose, watery eyes, and postnasal drainage—all of which began four days ago. Three days ago, he began monitoring his peak flow rates several times a day. His peak flow rates have ranged from 65-70% of his regular baseline with nighttime symptoms for 3 nights on the last week and often have been at the lower limit of that range in the morning. Three days ago, he also began to self-treat with frequent albuterol nebulizer therapy. He reports that usually his albuterol inhaler provides him with relief from his asthma symptoms, but this is no longer enough treatment for this asthmatic episode.
Case Study Questions
According to the case study information, how would you classify the severity of D.R. asthma attack?
Name the most common triggers for asthma in any given patients and specify in your answer which ones you consider applied to D.R. on the case study.
Based on your knowledge and your research, please explain the factors that might be the etiology of D.R. being an asthmatic patient.
Fluid, Electrolyte and Acid-Base Homeostasis:
Ms. Brown is a 70-year-old woman with type 2 diabetes mellitus who has been too ill to get out of bed for 2 days. She has had a severe cough and has been unable to eat or drink during this time. On admission, her laboratory values show the following:
Serum glucose 412 mg/dL
Serum sodium (Na+) 156 mEq/L
Serum potassium (K+) 5.6 mEq/L
Serum chloride (Cl–) 115 mEq/L
Arterial blood gases (ABGs): pH 7.30; PaCO2 32 mmHg; PaO2 70 mmHg; HCO3– 20 mEq/L
Case Study Questions
Based on Ms. Brown admission’s laboratory values, could you determine what type of water and electrolyte imbalance does she has?
Describe the signs and symptoms to the different types of water imbalance and described clinical manifestation she might exhibit with the potassium level she has.
In the specific case presented which would be the most appropriate treatment for Ms. Brown and why?
What the ABGs from Ms. Brown indicate regarding her acid-base imbalance?
Based on your readings and your research define and describe Anion Gaps and its clinical significance.
Submission Instructions:
You must complete both case studies
Your initial post should be at least 500 words per case study, formatted and cited in current APA style with support from at least 2 academic sources. -
