For this assessment, you will evaluate the preliminary care coordination plan you developed in Assessment 1 using best practices found in the literature.
This assessment provides an opportunity to research the literature and apply evidence to support what communication, teaching, and learning best practices are needed for a hypothetical patient with a selected health care problem.
NOTE: You are required to complete this assessment after Assessment 1 is successfully completed.
You are encouraged to complete the Vila Health: Cultural Competence activity prior to completing this assessment. Completing course activities before submitting your first attempt has been shown to make the difference between basic and proficient assessment.
In this assessment, you will evaluate the preliminary care coordination plan you developed in Assessment 1 using best practices found in the literature.
To prepare for your assessment, you will research the literature on your selected health care problem. You will describe the priorities that a care coordinator would establish when discussing the plan with a patient and family members. Epidemiology Graph
Instruction:
Choose an epidemiology graph of a chronic disease of your choice in a developing country from a peer reviewed resource.
Explain in one (1) paragraph why you chose that graph. A paragraph is minimum of three sentences.

Submission Instructions:
The paper is to be clear and concise and students will lose points for improper grammar, punctuation and misspelling.
The paper should be formatted per current APA and references should be current (published within last five years) scholarly journal articles or primary legal sources (statutes, court opinions.)
The paper is to be 4 pages in length, excluding the title and references page.
Incorporate a minimum of 3 current (published within last five years) scholarly journal articles or primary legal sources (statutes, court opinions) within your work. Journal articles and books should be referenced according to current APA style.

For this assignment, you will prepare a 3 to 4 page review of the literature pertinent to a selected problem for healthcare research and to use that review to propose a methodology to address the problem. Plus the evidence matrix table provided.
Conduct a search of literature relevant to the problem/topic. Identify a minimum of 4 references, most of which are randomized clinical trials. Only one opinion articles may be included.
Read the peer- reviewed articles with the focus of preparing a document that will compare and contrast the information in the articles you found. Copies of the articles used must be submitted with the final paper.
The reader of your literature review should be able to clearly identify the gaps in the knowledge in the problem area as well as the purpose of the study you are proposing. The number of pages in this assignment should be no longer than 4 pages. You should be able to write enough to create an effective argument but not so much that the result looks padded.

Creating and making effective professional presentations requires specific skills and proficiency, and one’s strengths and weaknesses influence the ability to present professional presentations effectively. One strength of mine in creating and delivering effective professional presentations is my ability to apply creativity, which enables me to grab the attention of my audience from the start to the end of the presentation; I use creativity to organize my presentation in a way that it flows and connects appropriately, and uses catchwords to gain the attention of my audience, get them interested in the presentation and engrossed in the presentation to the end. I also maintained good eye contact with the audience and occasionally tell jokes that connected with my presentation to interest them. Also, I use tone, pitch, voice, and volume that enable my audience to connect with the presentation and structure the presentation so that it flows (Hagler, 2022).
My weakness is that sometimes I get caught up in speaking anxiety, which sometimes can affect my presentation. One way to improve my strength is to continue engaging in more presentations, knowing that the more I engage in presentations, the more I will be able to perfect my creativity skills. Also, one way to address my weakness is to take time to rehearse my presentation and master the content prior to making the presentation (DiBartolomeo, 2022). Continuous engagement in presentations and practicing my presentation prior to making the presentation will go a long way in helping to perfect my presentation skills and make effective presentations in a formal setting (DiBartolomeo, 2022).
