This week, you will complete a journal focused on the social determinants of health in the U.S. For this journal, you are only focusing on five of them. This activity will help you understand how they harm society.
Within your own words, write a 500+ word single-spaced journal entry answering the following:
Identify and briefly discuss five social determinants of health. Do not list them. 
Explain why addressing the five social determinants of health you selected is essential to our society. Apply at least three social concepts and one social theory in your response.
Select three of the five social determinants of health.  Explain how they hurt the health outcomes of three minority populations (e.g., Asian American, Hispanic, African-American). Please be sure to identify the specific populations. For example, you might explain how employment, status, and where a person lives (3) impact the Hispanic population (one population).  You would do the same for Asian Americans (second population). You would explain how employment, status, and where Asian Americans live have an impact.    
Select one of the five social determinants of health and include three recommendations for addressing it. In other words, what are three suggestions you might make to decrease adverse health outcomes associated with lack of transportation – (improve public transportation accessibility, support community-based programs, promote other options such as walking) – DO NOT USE THIS EXAMPLE.
