Studies have demonstrated that these perceptions of these individuals can result in adverse effects, such as social rejection and avoidance actions. The individual may struggle to resist the influence of societal norms and attitudes (Milligan & Neufeldt, 2001). Economic status is a social factor that can impact disabled persons. These individuals are more likely to be unemployed and live in poverty (“Disability & Socioeconomic Status,” 2010). Understanding the client’s values and beliefs is essential as it can shape their treatment needs. These factors intertwine to shape the psychological and social experiences of individuals with disabilities, highlighting the importance of addressing both aspects in promoting their well-being and integration into society.

    While looking for an article for this week’s discussion board, I kept going back and reading the two (2) articles cited above. I found good points in each article that just stood out to me, and I couldn’t decide which one to use, so I decided to use both. Gilbert et al. (2013) talks about intrinsic motivation which is when someone does something just for the enjoyment of doing it (Oudeyer & Kaplan, 2007). This could go hand in hand with their lack of self-esteem or poor self-esteem. A person with a disability may feel less than others (Dong et al., 2022) so when they find something they enjoy doing for themselves, they do it for the self-esteem boost.
    Two factors you describe should be socially related.
    Having friends/coworkers that support them is critical to their success in life, both socially and in the workplace. Everyone needs support to get through the day, as person with a disability (physical or mental) just needs a little more. This could be a support group or accommodations to help their day go a little easier (Dong et al., 2022). Active listening is an important tool that everyone should use daily no matter who they are dealing with. When you use Active Listening, you let the person you are holding a conversation with that you care about what they have to say.

    I commonly use Active Listening everyday. With my husband, children, coworkers, clients, just about anyone I meet that would like to talk. SSRIs such as fluoxetine (Prozac) and sertraline (Zoloft) were frequently classed as Category C, suggesting that their safety during pregnancy had not been thoroughly proven and that their usage during pregnancy would depend on the situation (Fedrizzi S, A. 2023).  
    Risks and benefits of the off-label drug
    Off-label prescriptions involve the use of medications in ways that are not officially approved or indicated by regulatory authorities. This might include repurposing drugs for different uses, as well as using different dosages or durations than what is prescribed. Antipsychotics are used off-label during pregnancy for treatment as antiemetics or for sleep purposes. Cessation of off-label antipsychotic medications during pregnancy heightens the likelihood of relapse in depression and bipolar (McDonald M, & Alhusen J. A 2022).
    Clinical guidelines for prenatal depression
    According to McDonald M, & Alhusen J. A (2022), Perinatal depression affects around 1 in 7 women and is recognized as the most prevalent consequence of pregnancy and childbirth. A study revealed that maternal depression during pregnancy is correlated with an increased likelihood of preterm birth, small for gestational age, stillbirth, low birth weight, and maternal morbidity, which includes perinatal problems, higher rates of surgical delivery, and postpartum depression. To avoid these negative consequences, it is important to conduct screenings for depression, closely monitor individuals, and effectively manage the condition while taking into account the balance between potential risks and benefits. Addressing this problem during pregnancy is a multifaceted matter, necessitating healthcare providers to meticulously evaluate the advantages and drawbacks of medication.
    Healthcare practitioners should consider prevalent obstacles to treatment, such as insufficient screening and societal prejudice. Identifying and treating this issue can decrease the prevalence of suicides among perinatal women experiencing depression, as well as mitigate the potential negative impacts of untreated maternal depression on their child’s cognitive and behavioral growth. screening all pregnant and postpartum women for perinatal depression is now considered a reimbursable medical cost. Regular counseling sessions can effectively prevent perinatal depression in certain patients. Recently authorized parenteral drugs provide quick relief for symptoms in cases of mild to severe illness. Studies suggest that biomarkers, specifically epigenetically changed genes, could potentially be used to predict prenatal depression (Fedrizzi S, A. 2023).
    Food and Drug Administration developed a system that determines the teratogenic risk of drugs by considering the quality of data from animal and human studies. The FDA divides all medications taken during pregnancy into five groups: A, B, C, D, and X. Pregnancy is not advised in any circumstance to use category X products; category A is thought to be the safest category. This gives the clinician therapeutic direction. After controlling for confounding variables related to underlying psychiatric disease, SSRI drugs are not linked to greater incidents of birth abnormalities or long-term impairments in mental development. During pregnancy, SSRIs such as fluoxetine (Prozac), escitalopram (Lexapro), sertraline (Zoloft), and citalopram (Celexa) can be used as antidepressants. Premature birth and high blood pressure in the expectant mother are among the risks. These are not very big dangers. Throughout your prenatal care, your healthcare team keeps an eye out for these (Fedrizzi S, A. 2023). 
    Conclusion
    Early identification and intervention are crucial in addressing antenatal depression due to the potential risks it poses, including perinatal problems, heightened likelihood of surgical delivery, and the development of postpartum depression, which can adversely affect maternal well-being. The objective of perinatal mental health treatment is to effectively provide medication to alleviate the physical and psychological challenges caused by mothers’ psychiatric disorders. Engaging in regular monitoring of symptoms during pregnancy and after giving birth, as well as making necessary adjustments to medication dosage to maintain effectiveness, is considered a commendable approach.
    Depression during pregnancy is a prevalent condition, however, it is frequently not recognized due to the tendency to attribute its symptoms to the pregnancy itself. It is advisable to utilize selective serotonin reuptake inhibitor antidepressants, specifically fluoxetine, as they have not been linked to causing birth defects. Additionally, non-pharmacological treatments including psychotherapy, mindfulness, and aerobic exercise should be used. Providing education to healthcare providers will enhance their ability to accurately diagnose and treat this illness.

     As people in a highly visual world, we take in information and make judgments largely through our visual perceptions. But what about what cannot be seen? Invisible disabilities are just that, invisible. However, they may be recognizable to those who live with them. Invisible disabilities include such conditions as diabetes, autism, fibromyalgia, traumatic brain injury, and mental illness among others. These are neurological or physiological conditions that are not immediately apparent but that nevertheless affect every aspect of a person’s life. When discussing their disability with someone else, they may receive comments like, well, you look fine to me. Are you sure there’s something wrong with you? You don’t seem disabled? Oh, it’s all in your head. These kinds of statements invalidate their experience and the very real symptoms and challenges they may be having. So not only may the person feel marginalized by the disability itself but also by the lack of acknowledgment and empathy from others. Social workers recognize the impact of invisible disabilities in their assessments and treatment plans examining how the disability intersects with other factors to influence the client’s well-being. They also know that just because a disability is hidden it does not make a person any less worthy of support. 
    RAMON:
    Ramon
    Ramon is an 18-year-old male who identifies as Latino and African American. He is in his senior year of high school and lives with his parents and two sisters.
    Ramon has been diagnosed with an intellectual disability and with moderate hearing loss in his right ear. His mother, Angela, reached out to me, the school social worker.
    Background and History
    Early History
    After gaining consent, I met with Angela and Ramon for background and history. I asked about Ramon’s early history, and she reported that as a baby, Ramon had multiple ear infections as well as seizures. He was hospitalized and put on medication to control the seizures, though nothing was done for the ears. At 5 years old, he became very sick, and it was discovered that his Eustachian tubes were bent, which had resulted in muffled hearing on the right side. Angela reported that if the doctor, who was White, had noticed the issue when he’d been a baby, Ramon could have gotten tubes placed in his ears and would not need a hearing aid. Angela reported skepticism and distrust of the medical community based on this experience.
    School
    Angela reported that Ramon adapted over time to a hearing aid. He remained somewhat removed in social situations and was overwhelmed when he entered school. Because of his initial struggle in academics, his IQ was assessed and determined to be below average; throughout the duration of his public schooling, Ramon had an Individualized Education Plan (IEP) based on this identified intellectual disability.
    Presenting Issue
    Once I had this background from Angela, I asked what brought them here. Angela mentioned that the college and career counselor at the high school had implied that college was not in Ramon’s future. Angela was furious and did not feel heard by the school administration when she brought up the incident with them. She would like Ramon’s IQ to be reassessed. She felt that the original practitioner who performed the IQ assessment “did not get it right” and that “Ramon has grown so much.”
    Session With Ramon
    I then met with Ramon individually. During the meeting, I observed that he fidgeted and appeared anxious, occasionally biting his fingernails. “How long is this going to take?” he asked, while looking at the time. Ramon reported that he didn’t want to be late for gym class. He hung his head and talked in a low voice but responded to all of my questions appropriately. When I asked how he felt about college, he said he wanted to “go to LSU” and that the college and career counselor had started showing him job listings instead. “Now my mom’s all mad,” he said and rolled his eyes. “Are you mad?” I asked. Ramon said, “I don’t think that was fair of him to do. I’m not dumb.”

    I asked Ramon how high school had been for him. And he responded that he didn’t wear his hearing aid because it made him self-conscious around his peers. He sat on the right side of the classroom so he could hear clearly from his left ear. This provoked some anxiety prior to each class about whether he would be able to find a seat that would position him well to hear. In cases where he could not hear, Ramon could not fully participate in class; teachers interpreted this lack of engagement as part of him being “slow.” Ramon also reported fear and anxiety around test-taking and social interactions. Other than that, he explained that he generally did okay in school, and his grades back up this statement.
    Based on my recommendation, the school supported carrying out another assessment for both IQ and mental health. The findings were that Ramon does not have a low IQ but rather generalized and social anxiety disorders. These mental health issues were contributing to Ramon’s social isolation and feelings of extreme worry, which in turn impacted his performance on both the IQ assessment and school tests.

    In the Voices of Diversity video, it became evident that perspectives and experiences regarding ability and disability differ among individuals. Dr. Tammy Frye and Dr. Gina Bowlin shed light on the discomfort and challenges associated with discussing and understanding ability and disability. They emphasize that from a young age, individuals are often taught to avert their gaze or ignore people with disabilities, resulting in feelings of invisibility and marginalization for individuals with disabilities. I had such an experience growing up and was told to avoid disabled people especially those in wheelchairs, I was told that they get angry when stared at and to not stare at the blind when I see them which gives a rude vibe.  Dr. Frye shares her personal experience of acquiring a disability and feeling dismissed and in need of assistance, but people around her were not comfortable offering help. Dr. Bowlin adds that embracing and understanding differences can be difficult for many, but professionals in helping fields like social work may be more inclined to bridge the gap and support individuals with disabilities. Both speakers acknowledge the progress made in terms of providing accommodations and visibility for individuals with disabilities but acknowledge that there is still much work to be done (Walden University, LLC, 2021).
    Huang et al.’s scholarly resource on “Measuring Implicit Biases About Disability: A Scoping Review” is an article I read that explores deeper into topics of ability and disability. This article explores the notion of unconscious prejudices against disability and analyses available techniques for assessing these biases. The authors emphasize that individuals’ unconscious attitudes and beliefs have a substantial influence on how they view and interact with persons with disabilities. They contend that addressing and understanding these unconscious prejudices is critical to fostering diversity and eliminating discrimination. The article addresses the many measures used, such as the Implicit Association Test (IAT) and Priming Tasks, and advocates for greater study into developing more complete tools for detecting implicit biases connected to disability.
    The scoping review findings directly impact social work practice and are consistent with the ethical standards established in the NASW Code of Ethics (2021). As advocates for social justice, social workers address structural challenges such as disability discrimination. Dr. Frye’s emphasis on open communications is consistent with social workers’ dedication to knowing their clients’ abilities and impairments. Understanding hidden biases is essential for social workers to deliver fair and client-centered care. Dr. Bowlin’s appeal for education and cultural competency is consistent with social workers’ continual commitment to educating themselves on the various needs and obstacles that people with different abilities experience.

    Analyze what you learned from the Voices of Diversity video regarding perspectives and experiences related to ability and disability. 
    Overall, I enjoyed the Voices of Diversity very regarding perspectives and experiences related to ability and disability. Dr. Tami Fye discusses how from a young age people avoid looking at those with disabilities. She discusses how avoidance stems from discomfort that has been ingrained in people leading to a sense of indivisible for those with disabilities. Dr. Bowlin made a good point in her view on how many accommodations for disabilities are designed by those who are able-bodied. Both Dr. Fye and Dr Bowlin emphasize that cultural competence in addressing ability and disability is not a choice but critical in the social work field. As social workers we should be knowledgeable and well trained when working with all individuals.  
    Then, summarize your findings from at least one professional or scholarly resource focusing on themes of ability and disability. 
    The article I read was Making Disability Visible in Social Work Education by Kim, JaeRan Sellmaier, Claudia. The article discusses the role of social work education and how it relates to working with individuals with disabilities. In addition, the article discusses theoretical frameworks that can help social work programs make disability intentionally planning and teaching students across a spectrum of abilities (Kim, 2020). Considering that I have not worked with individuals with disabilities I felt this article was important to read. In order for social work programs to respond thoughtfully and fully to the spectrum of all students of all abilities we should be looking to “invite disability in the front door” by proactively creating a non-disabling program, rather than reacting to disability on an individual basis (Kim, 2020). In the article it also discussed how within the social work program it should be relatable to those with disabilities. As a former teacher I enjoyed reading this because in the school system many of curriculums aren’t appropriate for all individuals in the classroom.  
    How do your findings apply to social work practice with clients with varying abilities? 
    I believe that my findings apply to social work practice because as social workers we need to use practices that consider the diverse abilities of clients. The emphasis on advocacy in the article connects with the social work profession’s goal to promoting social justice. Social workers should actively advocate for the rights and needs of clients with disabilities, ensuring they have equal access to resources, opportunities, and services. Empowering clients to self-advocate and promoting their self-determination becomes essential.
