  • Chronic Health Condition: An Overview and Evidence-Based Plan for Nurse Practitioners Introduction: Chronic health conditions are a significant and growing problem in the United States, affecting millions of individuals and placing a significant burden on the healthcare system. These

    Introduction (Identification of the problem) with a clear presentation of the problem as well as the significance and a scholarly overview of the paper’s content. No heading is used for the Introduction per APA current edition.
    Background and Significance of the disease, to include: Definition, description, signs and symptoms. Incidence and prevalence of statistics by state with a comparison to national statistics pertaining to the disease. If after a search of the library and scholarly data bases, you are unable to find statistics for your home state, or other states, consider this a gap in the data and state as much in the body of the paper. For instance, you may state something like, “After an exhausting search of the scholarly data bases, this writer is unable to locate incidence and/or prevalence data for the state of…” This indicates a gap in surveillance that will be included in the “Plan” section of this paper.
    Surveillance and Reporting: Current surveillance methods and mandated reporting processes as related to the chronic health condition chosen should be specific.
    Epidemiological Analysis: Conduct a descriptive epidemiology analysis of the health condition. Be sure to include all of the 5 W’s: What, Who, Where, When, Why. Use details associated with all of the W’s, such as the “Who” which should include an analysis of the determinants of health. Include costs (both financial and social) associated with the disease or problem.
    Screening and Guidelines: Review how the disease is diagnosed and current national standards (guidelines). Pick one screening test (review Week 2 Discussion Board) and review its sensitivity, specificity, predictive value, and cost.
    Plan: Integrating evidence, provide a plan of how a nurse practitioner will address this chronic health condition after graduation. Provide three specific interventions that are based on the evidence and include how you will measure outcomes (how will you know that the interventions have utility, are useful?) Note:  Consider primary, secondary, and tertiary interventions as well as the integration of health policy advocacy efforts. All interventions should be based on evidence connected to a resource such as a scholarly piece of research.
    Summary/Conclusion: Conclude in a clear manner with a brief overview of the keys points from each section of the paper utilizing integration of resources.
    The paper should be formatted and organized into the following sections which focus on the chosen chronic health condition.
    Adhere to all paper preparation guidelines (see below).

While nursing studies have been conducted to empirically investigate factors that influence African American women in following the breast cancer screening guidelines each have concluded that a qualitative study is indicated (Cullen, 2007, Anderson, 2008 and Smith, 2009).
    Research Questions and/or Research Hypotheses
    A study can designate either to have research questions or research hypotheses. The researcher will decide which one is appropriate depending on the research problem statement.
    Research Questions:
    · Write the research question(s) clearly concisely.
    · The questions need to contain the population to be studied, the setting, and the variables of interest.
    · The relationship between the problem and the research questions needs to be clear and indisputable.
    · Connect the questions with the identified dependent and independent variables
    o Example: In African American women ages 30 to 65 living in the Southeastern region of the United States, how does lack of insurance affect having a mammogram?
    Research Hypotheses
    · Write each hypothesis to state the expected outcome and indicate how this will be measured.
    · You can write a simple hypothesis, which is a hypothesis that expresses an expected relationship between one independent and one dependent variable.
    · A complex hypothesis refers to a prediction of a relationship between two (or more) independent variables and/or two (or more) dependent variables.
    o Example: More African American women ages 30 to 45 who participate in a breast cancer education will get a mammogram than African American women ages 55 to 65 who receive only printed literature.
    Theoretical/Conceptual Framework
    In this section select a nursing or other theory that describes the problem being identified.
    · Describe fully the theoretical or conceptual framework you selected to guide the development of the study.
    · Explain the theory in sufficient detail to allow the reader to follow the logical flow between the theory and the proposed study.
    · Make a conceptual map identifying the linkage between the problem, the theory, the interventions and the expected outcomes.
    o Example: The theory that will be used to guide this research is Imogene King’s Theory of Goal Attainment (King,1981) because this theory states that individuals have the capacity to think, make choices, and select alterative courses of action that influence the goals they set and attain.

    • Follow the outline to ensure you are addressing a topic we have covered in class.
