Completing this assessment will strengthen your understanding of ethical issues and policies related to the coordination and continuum of care, and will empower you to be a stronger advocate and nursing professional.
    Preparation:
    It would be an excellent choice to complete the Vila Health: Ethical Decision Making activity prior to developing the presentation. The activity provides a helpful update on the ethical principles that will help with success in this assessment.
    Scenario
    Your nurse manager at the community care center is well connected and frequently speaks to a variety of community organizations and groups. She has noticed the good work you are doing in your new care coordination role and respects your speaking and presentation skills. Consequently, she thought that an opportunity to speak publicly about contemporary issues in care coordination would be beneficial for your career and has suggested reaching out to a community organization or support group to gauge their interest in hearing from you, as a care center representative, on a topic of interest to both you and your prospective audience. You have agreed that this is a good idea and have decided to research a community organization or support group that might be interested in learning about ethical and policy issues related to the coordination of care.
    To prepare for this assessment, you may wish to:
    Research your selected community organization or support group.
    Review the Code of Ethics for Nurses With Interpretive Statements and associated health policy issues, specifically, the ACA.

    Develop a PowerPoint with typed speaker notes (the script for your voice recording) and audio voice-over recording, intended for that audience. Video is not required.

    Nurses play a crucial role in
    patient care as primary caregivers, often the first to observe changes in a
    patient’s condition. Their ability to recognize and respond to early signs of
    deterioration is vital in preventing adverse outcomes(Mushta et al.,
    2018).However, failures to recognize such changes can occur, leading to serious
    complications. In a clinical setting, I witnessed a failure to recognize a patient’s
    deteriorating condition, leading to septic shock and prolonged ICU stay due to
    inadequate monitoring, lack of training, and communication breakdown. Deficits
    in clinical data analysis and cue recognition are compounded by novice skill
    levels, inadequate nursing education, and lack of experiential knowledge,
    further contributing to the problem of failure to recognize patient
    deterioration (Mushta et al., 2018). Early recognition and prompt intervention
    could have prevented the patient’s severe decline, as signs of patient
    deterioration are often observable hours before a cardiac arrest but may be
    subtle and overlooked by healthcare providers. According to Mushta et al.
    (2018), preventing such failures requires enhanced training on early signs of
    deterioration, improved monitoring protocols (including high-frequency nurse
    surveillance at least 12 times daily), and effective communication strategies
    to facilitate timely reporting and response to patient deterioration. Utilizing
    technology for continuous monitoring and fostering a supportive environment
    where nurses feel empowered to escalate concerns can significantly reduce the
    risk of failure to recognize and improve patient outcomes (Astier et al.,
    2020).

    Failure to Recognize
    Nurses spend more bedside time with patients than other treating
    practitioners in healthcare. The ability to critically assess and develop early
    recognition of symptoms that may lead to harmful complications and adverse
    outcomes is vital for appropriate and timely intervention (Mushta et al, 2018).
    Inexperience often leads to failure to recognize; however, it is important to
    consider that vague symptoms or similar clinical presentations of less severe
    differential diagnoses may also be to blame (Britt and Agarwal, 2023; Mushta et al, 2018). In a
    clinical rotation in the ER, a female 41-year-old patient was triaged with
    symptoms of severe headache following a workout at her gym. Additional symptoms
    were photosensitivity, hypertension, and nausea. Medical directives were
    related to the migraine presentation which consisted of analgesics and
    antiemetics. This patient’s symptoms later escalated to continued hypertension
    and vomiting. Upon physician assessment, a CT was ordered as a precaution,
    which revealed multifocal and vertebral artery dissections (VAD). VADs are rare, however are the leading cause of stroke in patients
    under 45, carrying contributing risk factors including hypertension, female
    gender, and physical exercise (Britt and Agarwal, 2023). Acute clinical
    presentation is noted to be severe headache, vomiting, and rarely neurological
    symptoms in the initial stages (Britt and Agarwal, 2023). Had this patient not
    received precautionary imaging by the physician, it is likely they would have
    been sent home with migraine medication, not receiving appropriate treatment
    and may have suffered a stroke or death (Britt and Agarwal, 2023). In this
    case, failure to recognize with the potential of significant patient harm was
    owed to imprecise symptoms leading to less severe differentials as opposed to
    nursing assessment inefficacy.

    Week 6 Lecture Notes
    Exploring the concepts of change gave us some knowledge of the dynamics involved in designing, communicating, implementing, and committing to change within the organizational structure. The final task in the change process is to understand the principles behind sustaining change and learning from the failures associated with change initiatives. It is useful to remember that sometimes a change process may not be quite as successful as expected and this possibility comes with the overall package. The actions taken post-failure or conflict will help determine the sustainability of the intended change movement.
    Leadership must be aware of and alert to the behavioral variables associated with change so that they are not manipulated by the cultural dynamics of the organization. Resistance to change and the desire to keep things as they were is a behavior typically displayed during the change process. Being open to strategic dialogue to address the fears will assist in moving in the right path of the needed change initiative.
    When a change process is unsuccessful in meeting the requirements specified, or totally fails, there is often a tendency to make hasty changes or abandon the initiative for another plan of action that only worsens the failure—into something even more catastrophic. Promptly tackling a change process that fell short allows the leader and the participants the opportunity to debrief and discover what went right, what went wrong, and the correct path for future implementation.
    Heath and Heath (2008) explore the concepts of why some ideas stick and others never seem to get off the ground. Is it, perhaps, the story that goes along with the idea that causes the initiative to stick? Telling a good story goes a long way and the leader who has not mastered the art of persuasive conversation may experience greater difficulty in moving the concept along. We are never too important to become part of the story or process. The ground-floor leader has a better chance of finding success and finding willing participants.
    With open communication on the impact of the change on all shareholders, the leader will be able to define the parameters that will assure the sustainability of the change operative. There are numerous reasons for change to occur within the organization. On understanding the reasons for the change, the participants are more likely to cooperate and support the necessary actions required to make the change stick.
    Changing the culture is not something that will happen overnight. The culture of the organization must be given the time and support necessary to allow the new behaviors to become part of the dynamics of the group. Individuals will support the concepts and follow that leader who is willing to make them part of the overall process. Understanding the “why” behind the change process is the catalyst for making the change possible and ultimately sustainable.
